The L.P. Johnson Family Health Center (FHC) is a comprehensive family medicine teaching facility associated with the University of Illinois College of Medicine at Rockford Family Medicine Residency Program.
The FHC is staffed with board-certified faculty physicians who supervise resident physicians. The presence of licensed nurse practitioners with advanced academic and clinical experience enhances delivery of quality care and education. Our staff speaks multiple languages.
LP Johnson Family Health Center has earned the highest national recognition for patient-centered care. FHC’s Patient-Centered Medical Home, Your Care, is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system.
We proudly offer university-level:
- General wellness, acute and chronic care for all ages
- Women’s health, including OB, endometrial biopsies, colposcopies, preventive and prenatal care
- Smoking cessation counseling
- Joint injections
- Minor wound care
- Behavioral health services
- On-site lab, EKG and stress testing
Hours of Operation
FHC is open Monday through Thursday, 9:00 a.m. - 8:00 p.m. and Friday, 9:00 a.m. to 5:00 p.m. Please call for an appointment. (Established patients can walk-in, without an appointment, Monday through Friday, between 9-4 for sick visits.)
