Multiple Sclerosis Prevalence Study Survey
The University of Illinois College of Medicine has received funding from the State of Illinois Excellence in Academic Medicine grants to study the prevalence of multiple sclerosis (MS) in 13 northwest Illinois counties: Boone, Bureau, Carroll, DeKalb, Henry, Jo Daviess, La Salle, Lee, Ogle, Rock Island, Stephenson, Whiteside and Winnebago. Through this study, we hope to find how many persons have MS. We also want to learn about the people who have these diseases, such as their age, how long they have lived in the community, and about their diagnosis of MS. Please take just a few minutes to complete this brief survey. Only adults 18 or older should complete the survey. Your answers are anonymous and will be combined with others' responses. No individual responses will be reported.
