University School Based Health Centers at South Beloit


Goal

Our goal is to improve physical and emotional wellness of students. These health services centers provide convenient access to health care to support personal and academic success.


Services

Much like traditional clinics, we provide:

  • Well child visits
  • School and sports physicals
  • Health screenings and immunizations
  • Lab tests
  • Treatment for minor illness and injuries
  • Medication management
  • Contraceptive care
  • Assistance and treatment with all health concerns
  • Guidance and counseling:
    • Mental health
    • Risk-taking behaviors
    • Family planning

 

We also assist with:

  • Insurance applications
  • Referrals for special needs
  • Coordination of care with a primary care provider
  • Translation services

 

When a student needs additional services not provided at the Center, we will make a referral. Referrals for care will be made based upon the student’s personal healthcare provider on record and their insurance provider.

Parent Involvement

Once permission is received, students will have access to care. All records are maintained in a manner consistent with a patient/provider relationship. While our staff respects the

patient/provider relationship, we encourage students to share their health information with their parents/guardians.

Appointments


To make an appointment, call us at 815.389.9252 during normal business hours.
If it is your first visit, please arrive 30 minutes before your scheduled visit to complete registration.  Necessary forms are provided below if you would like to print them and send them with your child at the visit or mail them to us at the address below.

Open to all students enrolled in South Beloit Schools. Blackhawk Elementary, Junior High and Senior High students have walk-in privileges. Clark and Riverview Elementary School students must be transported to us for services.
