The UI Health L.P. Johnson Family Health Center serves patients of all ages at a unique family practice that is a teaching facility associated with the University of Illinois College of Medicine Rockford.

Your health care team at the L.P. Johnson Family Health Center includes resident physicians in the Family Medicine Residency Program who get experience under the guidance of attending physicians. Our attending physicians are faculty members of the University of Illinois College of Medicine Rockford and oversee the care of patients at all phases of life. Nurses, medical assistants, a clinical pharmacist and behavioral health specialists are also part of your team, working together to provide the best possible care for you and your family.

We proudly offer a wide range of services for you and your family, including:

  • General wellness visits
  • School physicals and vaccinations
  • Care for non-emergency illnesses and injuries
  • Chronic condition care for all ages
  • Women’s health services, including pregnancy services, endometrial biopsies, colposcopies, preventive and prenatal care
  • Smoking cessation counseling
  • Joint injections
  • Minor wound care
  • Behavioral health services
  • On-site lab, EKG and stress testing

Hours of Operation
The L.P. Johnson Family Health Center’s normal hours of operation are:

Monday and Thursday, 8 a.m.-8 p.m.
Tuesday, Wednesday and Friday, 8 a.m. to 5 p.m.

Hospital Affiliations

Patients who see doctors at the L.P. Johnson Family Health Center can get care by that same doctor if they are admitted to SwedishAmerican Hospital: A Division of UW Health. For example, our expectant mothers can see an L.P. Johnson Family Health Center physician throughout their pregnancy, have that physician deliver their baby at SwedishAmerican Hospital, and then have that same physician care for both mom and baby in the hospital and back at the L.P. Johnson Family Health Center for follow-up visits. Our referral department also works closely with Mercyhealth, OSF Saint Anthony Medical Center and other health systems if you need specialty care or hospitalization as your insurance allows.
