The L.P. Johnson Family Health Center (FHC) is a comprehensive family medicine teaching facility associated with the University of Illinois College of Medicine Rockford Family Medicine Residency Program.
Staffed with board-certified faculty physicians who supervise resident physicians. The presence of licensed nurse practitioners with advanced academic and clinical experience enhances delivery of quality care and education. Our staff speaks multiple languages.
We proudly offer a wide range of services for you and your family, including:
- General wellness visits
- Care for non-emergency illnesses and injuries
- Chronic condition care for all ages
- Women’s health services, including pregnancy services, endometrial biopsies, colposcopies, preventive and prenatal care
- Smoking cessation counseling
- Joint injections
- Minor wound care
- Behavioral health services
- On-site lab, EKG and stress testing
Hours of Operation
The L.P. Johnson Family Health Center’s normal hours of operation are:
Monday through Thursday, 8 a.m.-8 p.m. and Friday, 9 a.m. to 5 p.m.

(Established patients can walk-in, without an appointment, Monday through Friday, between 9 a.m. and -4 p.m. for sick visits.)
