Any staff, resident, or faculty who test poitive for COVID-19 must notify Employee Health Services- Rockford at the time of the positive test and provide proof of the positive test. Proof of positive test can be from a providers office or home test. If using a home test please take a picture of the home test on top of a piece of paper with your name and the date of the test.
- Notify your immediate supervisor and follow department call-in procedures
- Notify Employee Health Services- Rockford (emphealthrock@uic.edu), include the following in your communication:
- Name
- Phone Number you can be reached at
- Reason and date for test (symptoms, exposure, etc)
- Date of symptom onset (if symptomatic)
- Proof of positive test
- Employee Health Services Rockford will contact you to obtain additional information needed
- Employee Health Services will send an email to your supervisor and Human Resources with the following information:
- Notification of positive COVID-19 test
- Return to on-campus work date
- Any restrictions including masking requirements
- Contact Human Resources if you have any leave benefit usage questions
COVID Return to Work Criteria
- 5 days after symptom onset or 5 days after positive test (whichever occurred first)
And
- Fever free (>/= 100.4 degrees) for at least 24 hour without fever reducing medications (Tylenol, Ibuprofen), symptoms resolving, and no wet symptoms (productive cough, runny nose).
And
- Must wear a mask through day 10
**COVID exposures do not require quarantine at this time
