LP Johnson Family Health Center to Close for Repairs on 2/11/16
The LP Johnson Family Health Center (FHC), located at 1221 E. State Street in Rockford, will be CLOSED on Thursday, February 11 as the City of Rockford makes repairs to the water main. We expect to resume normal hours of operation on Friday, February 12, 2016.
FHC's normal hours of operation: FHC is open Monday through Thursday, 9:00 a.m. - 8:00 p.m. and Friday, 9:00 a.m. to 5:00 p.m. Please call 815.972.1000 for an appointment. (Established patients can walk-in, without an appointment, Monday through Friday, between 9-4 for sick visits.)
NOTE: FHC WILL BE CLOSED THIS THURSDAY, FEBRUARY 11.
